Extra reports that the official cause of death has been released for Playboy founder, Hugh Hefner. The men's magazine publisher died at the age of 91 on September 27. According to reports, his death certificate, received by The Blast, says that Hefner had a heart attack and "respiratory failure." He also had septicema, a blood infection, and an "E. coli bacterial infection," which the death certificate notes did not respond well to antibiotics. Earlier reports say that Hefner died at The Playboy Mansion.
